• Choose a dental plan. The state of Texas offers three services including MCNA, Delta Dental, and Dentaquest. No matter which one you choose, Frisco Elm Dental provides services for each.

• Mail in forms or complete by phone. Once the above two steps have been completed, fill out the forms and mail them in or enroll by phone with Medicaid at 1-800-964-2777 or CHIP at 1-800-647-6558.
